OUR COLLABORATIVE PARTNERS

HTDI has established collaborative partnerships with other nonprofits, organizations, and agencies in an ongoing effort to support our mission to increase impact and provide more opportunities of H.O.P.E., for individuals and families, to access the supports they need to live independently. HTDI has a “revolving-door relationship,” with the following nonprofits and agencies, in which we refer our clients for some services, and accept referrals from our collaborative partners who have clients that are in need of our programs and services.

Caring Works, Inc.: Provides comprehensive case management and supportive service linkages to homeless persons with special needs.
Behavioral Health Link: Provides triage and intervention to individuals who are experiencing crisis.
St. Joseph’s Mercy Care Service: Provides Mental Health Case Management and HIV/AIDS education and testing.
St. Jude’s Recovery Center, Inc.: Provides critical care to individuals in Atlanta fighting homelessness and addiction. It is one of the few treatment center in Georgia that provides a full continuum of care.
In Partnership with Grady Hospital and Grady IDP Unit: Provides services to people with severe Mental Health/Substance Abuse and HIV/AIDS needs.
Aniz, Inc.: Promotes emotional and physical wellness by providing mental health and substance abuse counseling
AID Atlanta, Inc.: Provides persons living with HIV/AIDS with case management and medical resources.
AID Atlanta, Inc.: Provides persons living with HIV/AIDS with case management and medical resources.
AID Atlanta, Inc.: Provides persons living with HIV/AIDS with case management and medical resources.
